This is a new type of form below. It looks simple to maintain familiarity. It is in reality the front end to a secure database.

This form below uses the PHP programming language and it captures it results directly to a MySQL database. It does not generate e mail to the form owner like the old online forms did.. This was at first done done to infuriate malicious spammers who exploit forms. Then I began to see the potential of this type of form. The potential for this "capture to database technology" is to allow business and or organizations to maintain a full database of members where the task of data entry is maintained by the members who submit address and contact information updates to the database online, and all the secretary has to do is download the database and import it into MS Excel or MS Access. It is also an excellent way to mange a large library of business contacts. It can handle up to 20,000 records. The database is a de facto digital rolodex of business contacts that can be broken down by state and zip code. I call it "Web Based MySQL contact management" software. In reality I use it as an address book for my business and professional contacts.
